Making Arrangements

You may complete arrangements (whether at-need or pre-need) by simply selecting one of the following three options:

1. You may call our 24-hour telephone numbers listed below and schedule an appointment to come into our offices. (773) 736-2693 or (800) 375-6702
2. You may call our administrative offices, and we will fax you the necessary forms and authorizations with which to complete, and you may then fax them back to us. Our 24-hour telephone number is (773) 736-2693.
3. You may download the forms and authorizations necessary below. They are available in Abobe Acrobat Format. Kindly note that there are three documents:
   • Biographical Worksheet
   • Release of Remains
   • Cremation Authorization

Please note, all services must be performed by a licensed funeral director.  Rago Brothers Funeral Homes handles all arrangements and services for the American Cremation Society.  After you have completed and signed the necessary forms, fax them to our office at 773-589-1743.
